Remote Extended Care Management

Patient

Outside of their offices, physicians don't know what is happening with their patients. That's why Remote Care Management is becoming the "go to" program for seniors across the country. Remote care allows your physician to monitor your health on a continuing basis, so your medical care is always tailored to your needs. And it's covered by Medicare. (Copay may apply.)

MEDICARE & REMOTE CARE

It's no secret that for years Medicare has been struggling to contain costs while also improving patient care. Truly a tall order. So in 2015 they created an "experiment" in which they agreed to pay physicians for "remote care", in other words, for providing care via telephone, "Zoom" calls, etc., to augment office visits, to see if this helped in containing costs, while improving patient "outcomes". The results have been so overwhelmingly positive (see the results in the block above) that they are now promoting the program to physicians across the entire USA.

In early 2024 Medicare published the booklet shown on the above to let physicians know how dramatically Remote Care can benefit their patients, their medical practices, and their communities.

HOW IT WORKS

Your physician needs to be onboard with our program. If you don't know, just fill out the form below and we will contact your doctor's office.

Your physician works with our clinical team to determine which parts of the Remote Care program, if any, are best for you.

Our clinical team will contact you and explain the details of the service(s) that your physician agrees will be of most benefit to keeping you healthy.

Nothing happens until you fully understand the programs and agree to participate.

The clinician assigned to your remote care contacts you and begins the program to keep you healthy and out of the hospital.
