Option One: Sign our (standard) BAA and then enroll your patients using the tools you will be given. Your patients will be given a comprehensive assessment to determine their starting levels. If you choose, you will be able to see patients’ results online.
Option Two: Sign the CereSkills Administrative Services Agreement and the BAA, agreeing to a perform a brief monthly review of your patients’ progress and you will earn a monthly fee-for-service for your time.

Enroll Your Patient
We provide a fax number for your EHR System so you can send the face sheet the same way you do other referrals, when the patient shows an interest, and we contact the patient.

Patient Gets Started
Patients receive a few emails. to get started.
Register: Create the account and download the app.
Evaluation: Take assessment so the program can be customized to their needs and goals.
10 minutes a day your patient goes on their phone or tablet and perform basic movements that were customized to their evaluation and physical abilities.
Monthly, a coach will evaluate their progress and adjust the program as needed based on successes and areas of improvement still needed.
